Which Sugar Substitute Is Actually Healthiest? Six, Ranked by the Data

Dates, maple, coconut sugar, honey, cane sugar and agave ranked on glycemic index, fructose content and fiber. Dates come out ahead because the fiber is still attached — and agave, marketed hardest on its low glycemic index, comes out last for a reason worth understanding.

Some genuinely are — but the ranking is not the one the marketing implies, and the sweetener sold hardest as the healthy choice finishes last.

Here is the short version before the detail. The only sweetener on the shelf with a genuinely different metabolic profile is the one that isn't really a sweetener: whole dates and date sugar, which are dried fruit with the fiber still in them. Maple syrup and honey are real foods carrying trace minerals, but the sugar in them is free sugar and behaves like it. Coconut sugar sits in the middle — genuinely better than table sugar, though not by the margin its packaging claims. And agave — marketed hardest of all on its low GI — is roughly 85% fructose, which is more than high-fructose corn syrup, and fructose is precisely the sugar a controlled trial showed doubles the liver's fat production.

That last one is the inversion worth remembering: the sweetener with the lowest glycemic index in this group is arguably the one with the weakest case, because glycemic index only measures blood glucose, and fructose barely raises blood glucose because the liver takes it instead.

Why glycemic index is the wrong yardstick on its own

Glycemic index measures one thing: how much a food raises blood glucose in the two hours after eating it. That is genuinely useful information, and it is also incomplete in a way the sweetener market has learned to exploit.

Fructose does not raise blood glucose much. It therefore scores a low glycemic index. But the reason it scores low is not that the body handles it gently — it is that fructose is routed largely to the liver instead of circulating as blood sugar. A low GI here reflects a different metabolic destination, not a gentler one.

This is where the controlled evidence matters. In a double-blind trial, 94 adults drank 80 grams of sugar daily for seven weeks — fructose, sucrose, glucose, or none. Fructose and sucrose roughly doubled the liver's production of new fat. The same dose of glucose did nothing to it. So a sweetener marketed as "low glycemic" because it is mostly fructose is being sold on a technicality. Agave is the clearest case.

What actually makes dates different

Dates are the exception, and it is worth being precise about why rather than treating them as a health food.

They are more than 70% sugar. Nobody should pretend otherwise, and any recipe telling you a date-sweetened dessert is sugar-free is selling something.

And yet: across five varieties tested in both healthy and type 2 diabetic subjects, dates measured a glycemic index of roughly 44 to 55 — low-to-medium — with no significant post-meal glucose excursions in the diabetic group. The mechanism isn't mysterious. The fiber the fruit came with is still present, and it slows the rate at which those sugars arrive.

The critical distinction, which almost nothing on a supermarket shelf makes clear:

  • Date sugar is dehydrated whole dates, milled. The fiber is still there. It behaves like the fruit.
  • Date syrup has had the fiber strained out. It behaves like a syrup.

Same fruit, different product, genuinely different result. If a label says syrup, the advantage is gone.

What about the artificial ones?

Every sweetener above is caloric and comes from a plant. The other option is the synthetic ones — aspartame, sucralose, acesulfame K, saccharin — usually assumed to be the safer choice because they carry no sugar and no calories.

The largest direct look at that question does not support the assumption. Debras and colleagues followed 103,388 adults in the French NutriNet-Santé cohort across 904,206 person-years, measuring artificial sweetener intake from every dietary source — beverages, tabletop packets, dairy — using repeated 24-hour dietary records that captured brand names rather than relying on memory. Published in The BMJ in 2022, the analysis found total artificial sweetener intake associated with increased risk of cardiovascular disease overall and of cerebrovascular events. Broken out by molecule: aspartame tracked with cerebrovascular risk, while acesulfame K and sucralose tracked with coronary heart disease risk (Debras et al., BMJ, 2022;378:e071204).

Grade it honestly: this is an observational cohort, not a trial. It cannot establish cause, and reverse causation is a live concern — people already at higher metabolic risk are likelier to have switched to sweeteners in the first place, which can make the sweetener look responsible for a risk that preceded it. The cohort was also 80% women and skewed health-conscious, so it is not a cross-section of everyone.

What it does establish is that the confident version of the swap — that synthetic sweeteners are the obviously safer choice — is not backed by the best available data on people. At minimum the question is open, and the burden sits with the claim that they are safer, not against it.

Which lands where sensible eating usually lands: real ingredients that grew somewhere, in moderation, with the quantity as the thing you actually control.

What about blood pressure?

Worth separating carefully, because the evidence here is much weaker than the glucose evidence and gets quoted as though it were stronger.

The well-supported chain is: sugar-sweetened beverages → weight gain and metabolic changes → cardiovascular risk factors including blood pressure. That is largely observational, and it concerns total sugar intake and the beverages carrying it — not the choice between maple and cane sugar.

What is not established is that swapping one caloric sweetener for another meaningfully changes blood pressure. There is no good trial evidence that a person eating the same quantity of sugar from coconut rather than cane will see a different blood pressure reading. Anyone claiming a specific sweetener lowers blood pressure is well past the data.

The honest summary: quantity is doing nearly all the work here; sweetener choice is a second-order effect, and only dates change the picture much at all.

What we do in our recipes, and why

We sweeten with whole dates where the texture allows it, and with small measured amounts of maple or coconut sugar where it does not. Those are real decisions with real reasoning behind them, and they are also modest.

Two things we will not tell you:

That our sugar is real and theirs is artificial. It isn't. Starbucks' Classic Syrup is sugar and water; Cinnabon and Swiss Miss use real cane sugar. The difference between a commercial product and a home version is the quantity and what the sugar arrives inside — not real versus fake. Claiming otherwise is the easiest possible thing for a critic to disprove, and we would rather make the argument that survives.

That natural sweeteners are metabolically free. FDA counts dates, maple, honey, coconut sugar and agave as added sugars, and so do we — which is why every recipe on this site prints its added-sugar figure and its total-sugar figure, including the ones where the total is not flattering.

The claim we will make: less sugar, and where the recipe allows, sugar that still has its fiber. That one is true.
