Nature-First Wellness: The $6.8 Trillion Industry and the 120 Minutes That Actually Move the Needle
The global wellness economy hit $6.8 trillion in 2024 while a third of adults still don't move enough. The best-evidenced intervention costs nothing: about two hours a week outdoors.
The Global Wellness Institute's 2025 Global Wellness Economy Monitor put the wellness economy at $6.8 trillion in 2024 — nearly four times the size of the global pharmaceutical industry, and on track for $9.8 trillion by 2029. Over the same stretch, WHO researchers reported in The Lancet Global Health that the share of adults worldwide getting too little physical activity climbed from 23.4% in 2000 to 31.3% in 2022. Spending went up. The baseline went down. That gap is the most important fact in consumer health right now, and closing it turns out to depend less on what you buy than on where you spend two hours a week.

What $6.8 trillion buys, and what it doesn't
The wellness market is not one thing. The Global Wellness Institute's 2025 monitor breaks out healthy eating, nutrition and weight loss at $1,148.0 billion in 2024 and physical activity at $1,143.9 billion, with six sectors each projected to clear $1 trillion by 2029. Plenty of that spending is on things that work — gyms, whole foods, mental health services.
But a large share of it is spent on the promise of a shortcut, and the best test case is the most popular supplement in America. The Council for Responsible Nutrition's 2024 consumer survey, conducted by Ipsos, found 75% of Americans use dietary supplements, most commonly for general health, immune support (30%), energy (27%), and skin, hair and nails (26%).
Then in June 2024, Loftfield and colleagues published the largest analysis of the question in JAMA Network Open: 390,124 adults across three prospective U.S. cohorts, followed for up to 27 years, with 164,762 deaths recorded. Daily multivitamin users showed no lower risk of death from any cause than non-users — and no difference in mortality from cancer, heart disease, or cerebrovascular disease. The finding held in both the early and later halves of follow-up, which rules out the usual objection that sick people start taking vitamins.
This isn't an argument that supplements are useless; targeted repletion of a documented deficiency is a different question entirely. It's an argument about sequencing. The most-purchased intervention in the country does not move the outcome that matters most, while the intervention with strong dose-response evidence is free.
The 120-minute threshold
In 2019, Mathew White and colleagues at the University of Exeter analyzed responses from 19,806 adults in England's Monitor of Engagement with the Natural Environment survey (2014/15–2015/16), published in Scientific Reports. They asked a question nobody had answered cleanly: how much nature contact is associated with feeling well?
The answer had a shape. Below two hours a week, nothing. At and above it, a clear and sustained association.

Two details make this unusually actionable. First, benefits peaked in the 200–300 minute range with no further gain beyond it — this is a threshold, not a race. Second, the authors found it did not matter how the 120 minutes was accumulated: one long visit, two hour-long visits, or several short ones produced no difference in outcomes.
This is observational data, so it cannot prove that going outside causes good health rather than the reverse. But it sets a target that costs nothing and carries no downside: roughly 17 minutes a day, or a single long weekend walk.
What outdoor time does that a capsule can't
Three mechanisms have direct experimental support.
Light. In 2022, an expert consensus published in PLOS Biology by Brown and colleagues recommended a minimum daytime exposure of 250 lux melanopic equivalent daylight illuminance at eye level, a maximum of 10 lux in the three hours before bed, and under 1 lux during sleep. The same paper notes that standard office lighting typically delivers under 200 melanopic lux — below the daytime floor. A daylight overcast sky supplies more circadian signal in ten minutes outdoors than a full morning at a desk. That matters for the 30.5% of U.S. adults sleeping under seven hours, per the CDC's 2024 data, because daytime light is the primary input that anchors nighttime sleep.
Stress physiology. Antonelli and colleagues screened 971 articles for their 2019 systematic review and meta-analysis in the International Journal of Biometeorology, including 22 studies in the review and 8 in the meta-analysis. Salivary cortisol levels were significantly lower in forest-exposure groups than in urban comparison groups, and in all but two of the included studies, forest groups showed either lower post-intervention cortisol than controls or a significant pre-to-post reduction.
Rumination. In 2015, Bratman and colleagues published a controlled experiment in PNAS: a 90-minute walk in a natural setting reduced both self-reported rumination and neural activity in the subgenual prefrontal cortex, a region associated with risk for mental illness. A 90-minute walk in an urban setting did neither. Same duration, same effort, different environment, different outcome.
The clinical version of all this has now been tested. A 2023 systematic review and meta-analysis in The Lancet Planetary Health examined "nature prescription" programs against control conditions and found a mean reduction of 4.82 mmHg in systolic blood pressure (95% CI −8.92 to −0.72) and 3.82 mmHg diastolic (−6.47 to −1.16), plus a moderate-to-large effect on depression scores (SMD −0.50, −0.84 to −0.16) and improvements in anxiety.
Greener neighborhoods, lower mortality
The largest-scale evidence comes from where you live rather than what you do. A 2019 systematic review and meta-analysis in The Lancet Planetary Health, produced for a WHO health impact assessment, pooled nine longitudinal cohort studies across seven countries and more than eight million people. For each increment of 0.1 in NDVI — a satellite-derived measure of vegetation density — within 500 meters of home, all-cause mortality risk fell with a pooled hazard ratio of 0.96 (95% CI 0.94–0.97). Seven of the nine studies found a significant inverse association; two found none.
A 4% relative reduction per greenness increment is not a miracle. Applied across a city's population, it is a public health effect larger than most consumer products will ever deliver — and it accrues to people who did nothing but live near trees.
Where the actual deficits are
Nature-first isn't a rejection of modern medicine. It's a claim about where the unmet need sits. The measured deficits in the U.S. and globally are unglamorous and largely environmental:
- Time outdoors. The EPA-funded National Human Activity Pattern Survey found Americans spend an average of 87% of their time inside buildings and another 6% inside vehicles — roughly 93% enclosed.
- Movement. WHO's 2024 analysis in The Lancet Global Health, drawing on 507 surveys and 5.7 million participants, put insufficient physical activity at 31.3% of adults — about 1.8 billion people — and projected 34.7% by 2030 on current trends. - Sleep. 30.5% of U.S. adults slept under seven hours in 2024, according to the CDC's National Center for Health Statistics.
- Vitamin D. An analysis of NHANES 2007–2016 data covering 11,119 U.S. adults aged 50–79, published in Frontiers in Nutrition in 2023, found 4.6% severely deficient, 15.2% moderately deficient, and 33.6% insufficient — a pattern that tracks with indoor living, and one where testing before supplementing is the sensible order.
None of these are solved by a purchase. All four are improved by the same set of defaults.
A nature-first week that costs nothing
Specific enough to schedule:
- 20 minutes of outdoor morning light, ideally within an hour of waking. Overcast counts. This is the highest-leverage single change for sleep timing, given how far indoor lighting falls below the 250-lux daytime recommendation.
- Hit 120 minutes a week outdoors, accumulated however suits you — the 2019 data found no advantage to any particular pattern. Aim toward the 200–300 minute band where the associations peaked, then stop optimizing.
- Move one existing activity outside rather than adding a new one. The phone call, the coffee, the after-dinner decompression. Substitution beats addition for adherence.
- Take the 90-minute version once a week if you can — a longer walk in a genuinely green setting, which is the protocol that reduced rumination in the 2015 PNAS experiment.
- Fix the evening end too. Keep the three hours before bed under 10 lux of melanopic light. Dim, warm, low.
- Test before you supplement. Given ~20% deficiency prevalence in older U.S. adults, vitamin D is worth a blood test — and the multivitamin data suggests the default daily pill isn't buying what most people think it is.

Compounding beats converting
Quick fixes sell because they promise conversion — a switch flipped, a new person. The evidence here describes something less dramatic and more reliable: small environmental exposures, repeated, that show up as a 4% mortality difference in a satellite dataset or five points of systolic blood pressure in a meta-analysis. Nothing about that will feel like a transformation on any given Tuesday. That's what compounding looks like from the inside.
