What a Pumpkin Spice Latte Actually Does to Your Body
A Grande PSL carries 32 grams of added sugar, 150 mg of caffeine and 390 calories. Here is what each of those numbers does, what the evidence supports, and what gets claimed about them that it doesn't.
A Grande (16 oz) Starbucks Pumpkin Spice Latte made with 2% milk contains 390 calories, 50 grams of total sugar — about 32 of them added — and 150 mg of caffeine. In practical terms: more added sugar than the American Heart Association's entire daily limit for women, roughly a meal's worth of calories, and enough caffeine that a mid-afternoon one is still measurably in your system at bedtime.
That is the short answer. What follows is what each of those three numbers actually does once it is in you, which claims about the drink hold up, and which popular ones — including one about pumpkin and one about blood sugar — do not.

The sugar: 32 grams, and where it goes
The Center for Science in the Public Interest's 2025 analysis of Starbucks nutrition data puts a Grande PSL with 2% milk at 50 grams of total sugar, roughly 32 of which are added — about seven and a half teaspoons. The remaining ~18 grams is lactose, naturally present in the milk.

That split matters and it is the part most coverage skips. The lactose is not the problem — it comes with the protein and calcium of the milk, and FDA does not count it as added sugar. The 32 grams above it are the ones a barista pumped in, and they alone exceed the AHA's entire daily ceiling for women by 28%.
That distinction matters because the guidance is written about added sugar. The AHA's daily limits are 25 grams for women and 36 for men. One Grande is 128% of a woman's entire daily allowance and 89% of a man's, before anything else she eats that day. The 2025–2030 Dietary Guidelines for Americans keep added sugars under 10% of daily calories and add per-occasion framing around roughly 10 grams per meal or snack.
Most of that sugar is not in the espresso or the milk. It is in the pumpkin spice sauce, whose published ingredients lead with sugar and condensed skim milk. The syrup is the drink's sugar content almost in its entirety, which is why the number moves so much when you change the number of pumps rather than the size.

Removing whipped cream cuts fat and calories but barely touches sugar. Cutting pumps is the only change that moves the sugar line meaningfully, because the sugar is the sauce.
The caffeine: 150 mg, and how long it stays
A Grande PSL is built on two espresso shots, about 150 mg of caffeine — roughly a cup and a half of brewed coffee. The FDA cites 400 mg per day as an amount not generally associated with negative effects in healthy adults, so one PSL is well inside that.
The more useful number is the half-life. Caffeine's elimination half-life in healthy adults averages around five hours, with a wide individual range driven partly by genetics, liver enzyme activity, smoking status and oral contraceptive use. Run the arithmetic on a 3 p.m. PSL:

At 11 p.m. you are still carrying somewhere around 45 mg — about half a cup of coffee's worth — which is the mechanism behind the very common experience of sleeping poorly and not connecting it to a drink consumed eight hours earlier. Controlled work has found caffeine taken even six hours before bed measurably reduces total sleep time. If you are a slow metabolizer, the curve above is optimistic.
Whether that matters is individual. The point is that it is a knowable curve, not a mystery, and the afternoon PSL is the version of this drink most likely to cost you something you did not intend to spend.
Does a pumpkin spice latte spike your blood sugar?
Yes — it is 50 grams of sugar in liquid form with no fiber. But the interesting part is which version of that claim is actually supported, because the internet has settled on a stronger one than the evidence carries.
Well supported: the epidemiology. Malik and colleagues, publishing in Diabetes Care in 2010, pooled 310,819 participants and 15,043 cases and found that people in the highest category of sugar-sweetened beverage intake — most often one to two servings a day — had a 26% greater risk of developing type 2 diabetes than those consuming none or almost none. Later dose-response work puts the increase at roughly 27% per additional daily serving. A daily PSL through the season is a sugar-sweetened beverage habit, whatever else it is.
Also well supported: what you eat around it changes the curve. Shukla and colleagues gave the same meal to adults with type 2 diabetes in two different orders a week apart and found that when protein and vegetables came before the carbohydrate, mean glucose fell 36.7% at 60 minutes and the incremental area under the curve across two hours was 73% lower. The original study was 11 subjects, and the group has since extended it to prediabetes; other teams have replicated the direction in gestational diabetes. A PSL after lunch is a different event from a PSL as breakfast on an empty stomach.
Not well supported, despite being everywhere: that liquid sugar inherently produces a bigger glucose spike than the same sugar in solid food. When researchers match macronutrients and compare liquid to solid meals directly, postprandial glucose responses come out broadly similar. What liquids do reliably is deliver calories with less satiety — you do not stop drinking a PSL the way you would stop eating 50 grams of sugar in cake form — and that is the more defensible reason to treat a sugary drink differently from a dessert.
With over-the-counter continuous glucose monitors now widely available — Dexcom's Stelo was cleared in March 2024, Abbott's Lingo and Libre Rio in June — a lot of people are watching this in real time for the first time. Worth knowing before you interpret a curve: a post-meal rise is normal physiology, not damage, and a single-sensor reading on a single day is not a diagnosis. Lingo is not FDA-cleared for use in diabetes at all; it is sold as a wellness device.
Is there real pumpkin in it?
Yes, since 2015 — and the myth that there isn't has outlived the fact by a decade.
The original 2003 formulation contained no pumpkin. After sustained public criticism, Starbucks reformulated in August 2015 to include real pumpkin purée and to remove artificial coloring. The sauce's published ingredients list sugar and condensed skim milk ahead of pumpkin purée, so the pumpkin is present but minor by weight.
The honest read: "there's no pumpkin in it" is out of date, and "it has real pumpkin" is technically true but implies more than the ingredient order supports. The nutrition case against the drink was never about the pumpkin. It is about the 32 grams of sugar the pumpkin is suspended in, and reformulating the purée did not change that.
So is a pumpkin spice latte bad for you?
One is a dessert with caffeine in it. That framing settles most of the question.
Nothing about a 390-calorie sweetened drink is harmful in isolation, and treating a seasonal ritual as a moral failure is both wrong and counterproductive. What the data supports is a distinction between occasional and habitual: the diabetes-risk association in the Malik analysis is about daily intake, not an annual first-day-of-fall drink. Sixteen PSLs across a season and sixteen PSLs across sixteen days are different exposures with the same receipt.
For context on the category, CDC's National Center for Health Statistics reports ultra-processed foods supply 53.0% of calories for US adults. A sweetened seasonal latte is a small, visible instance of a much larger pattern — which is why the useful response is usually to change the drink rather than to quit it.
Three changes, in order of how much they move the numbers:
- Fewer pumps. Halving the sauce takes a Grande from about 50 grams of sugar to about 34. Nothing else you can order changes the number that much.
- Smaller size. A Tall is 12 ounces instead of 16, and the pump count drops with it.
- Earlier in the day. This costs nothing and addresses the caffeine curve above, which is the part of the drink most likely to affect you tonight.

A Grande PSL with 2% milk has about 50 grams of total sugar; a 12-ounce can of regular cola has about 39 grams. So the latte carries more sugar than a can of soda, though roughly 18 grams of the PSL's total is lactose from milk rather than added sugar. On the added-sugar line specifically — 32 grams versus about 39 — the soda is slightly higher. Both exceed the AHA's daily limit for women in a single serving.
Does the pumpkin spice latte have more caffeine than regular coffee?
No. A Grande PSL is built on two espresso shots at about 150 mg of caffeine. A Grande brewed Pike Place at Starbucks runs substantially higher — brewed coffee is generally more caffeinated per ounce than espresso-based milk drinks, because the milk displaces volume. If caffeine is what you are after, drip coffee delivers more of it per dollar and per calorie.
Can I drink a pumpkin spice latte if I'm managing blood sugar?
That is a question for your clinician or dietitian, not a recipe article, and the answer depends on your medications, your targets and the rest of your day. What is generally true: the sugar is concentrated in the sauce, so pump count is the lever with the most effect; and published food-order research suggests eating protein and fiber before a carbohydrate load meaningfully reduces the glucose excursion. Neither of those is a recommendation about your care.
Is the pumpkin spice latte vegan or dairy-free?
The pumpkin spice sauce contains condensed skim milk, so the drink is not dairy-free even when ordered with oat, almond or soy milk. Starbucks' published allergen information is the authority on this and it changes between seasons, so check the current listing rather than assuming.
